Please register for COVID vaccine if you are 5 years and older.
Pharmacy team will contact with you to finalize your appointment.
When you come to pharmacy for your appointment you can print and fill out COVID-19 vaccine consent form to shorten your time during appointment.
You can also get that printed in the pharmacy.
Please make sure to follow social distancing and wear mask during your stay in the pharmacy.

Currently NOT experiencing any of the following symptoms: fever, chills, cough or barking cough (croup), shortness of breath, sore throat, difficulty swallowing, runny or stuffy/congested nose, decrease or loss of taste or smell, pink eye, headache, digestive issues like nausea/vomiting, diarrhea, stomach pain, muscle aches, extreme tiredness, falling-down.
In the last 14 days NOT been advised to quarantine per the federal quarantine requirements.
In the last 14 days, have NOT been identified as a “close contact” of
someone who currently has COVID-19.
In the last 14 days have NOT received a COVID-19 Alert exposure
notification on cellphone.
Doctor, health care provider, or public health unit did NOT tell you
that you should currently be isolating (staying at home).
NOT over the age of 70 and experiencing any of the following: delirium, unexplained or increased number of falls, acute functional decline, worsening chronic conditions?

Please mention in special message section details of your symptoms or the reason for your eligibility for Public Funded Test.
Please write full information and name should match with health card. Otherwise we will not be able to make appointment. If you don't have health card, then use other photo ID.
At the time of your appointment please call us from parking lot and we will instruct you when to come in.
Private Funded (Travel purpose):
